I've lived in Mexico for more than 19 years and never once have I needed to provide my mother's maiden name on any form or in person. Because Mexicans have two surnames--usually the father's followed by the mother's--most forms have a final space for the mother's, which I leave blank. Thus I find it amusing to be addressed as "Sra. Frances" (my middle name) instead of "Sra. Willard," which is actually my father's first name. (Legal, albeit a long story.)
