I left from ZLO. I got the antigen nose swab test before my flight that day. Results took 30 minutes, 450 pesos. I filled out a short questionnaire at the lab there. I was also asked to fill out another health questionnaire in the airport at a separate table that was set up.
I brought both these documents to the ticketing agent who looked at the test result form and handed it back to me. I checked my bag to LA, which was not my final destination but I had to overnight there.
On arrival in LAX I went through a busy immigration line. I wasn't asked for any Covid related documents. The agent asked me if I had been exposed to the virus. I didn't go through customs which I thought was strange.
The next day I took my short flight to my final Ca destination.
Return trip - I was told I needed to have a Covid test so got one the day before my flight at a kiosk set up at Oakland airport. Free, self nose swab. It was convenient for me to get it at this location, although I was flying out of SFO the next day. I easily set up my appt online.
I read on the Alaska website, and was told by the Alaska agent at the gate in SFO, that I would need to complete a health questionnaire for my arrival in Puerto Vallarta. This form was available by code on my iPhone, and after several attempts I completed it.
My flight was an unusual direct flight from SFO to PVR. I was not asked at any time by anyone to produce either results of my test, or the health questionnaire.
As Kurt Vonnegut wrote, "Go figure".
